Household Employer Publications and Forms
As a household employer you may have tax liabilities. Get the IRS Publication 926
to read
 about your responsibilities. Get the IRS Form SS-4 to file for an employer
number. (If you are a subscriber to the PTO Payroll Service you
do not need to do this.)
PTO Hiring Forms
This packet of forms includes various documents that you may find useful as you go
through the hiring process for caregivers. These forms are specifically designed for
caregivers and will help you feel more comfortable with the process. These forms are
only available for use by PTOFamily members of our Hot
Jobs!, Sitter Connection, or
Payroll Service. If you are a member of one of these plans, please feel free to
download and use these forms as much as you need, you may copy or otherwise reproduce
these forms only so long as our copyright remains on the form. Per your contract
with us, you may not give these forms to anyone else without our express written
permission, doing so will violate your contract and terminate our services.
PTO Baby-Sitter Forms
 These two forms can be copied and used whenever you have a temporary or hourly
baby-sitter in your home. These forms are only available for use by PTOFamily
members of our Sitter Connection, Hot Jobs!, CarePremium or Payroll Service. If you are a
member of one of these plans, please feel free to download and use these forms as much as
you need, you may copy or otherwise reproduce these forms only so long as our copyright

Income Tax Withholding for Payroll
If you are using a payroll service or are offering income tax withholding your
caregiver will need to fill out a W-4 for federal withholding and an Arizona Withholding
form for state.
I-9 Immigration Form
 If you are the employer of your care-giver you may be required to fill-out and retain
the I-9, Immigration form. Please see the instructions included with the form for
information on how to use the form and which documents you can use to satisfy the
requirements.
